Privacy Policy
Trac-B Exchange provides harm reduction & substance use disorder services to individuals who are actively seeking help for treatment, as well as, using syringes for injecting drugs, steroids, hormones, insulin, etc. and are at high risk for exposure to Hepatitis C and HIV. It is Trac-B Exchange’s policy to offer participants confidential syringe exchange, peer recovery support, and testing services and maintain the confidentiality of all participant personal information.
Participants are required to provide identifying personal information to receive certain services from Trac-B Exchange. During the course of service provision, Trac-B Exchange staff, consultants, interns and volunteers may learn confidential participant health information and/or other personal or identifying information (“Confidential Information”).
No Trac-B Exchange staff, consultant, intern or volunteer shall disclose participant confidential information to any third-party without the express written consent of the participant, except for in the certain limited circumstances .
Federal and state law requires Trac-B Exchange to maintain the privacy of your Confidential Information.
It also requires that you be notified of your rights, our privacy practices, and any other legal obligations concerning your Confidential Information.
Trac-B Exchange reserves the right to make changes in our Privacy Practices at any time.
Upon any changes to our practices, we will issue a new Notice of Privacy Practices which will be conspicuously posted in Trac-B Exchange’s offices, with copies available upon request.
Trac-B Exchange will not disclose your Confidential Information to any third-party without your express written consent except in the following situations:
In an Emergency: Trac-B Exchange may use or disclose your Confidential Information in the event of a medical emergency in which Trac-B Exchange staff, interns, consultants, or volunteers reasonably believe that such disclosure may save your life (i.e.: overdose on Trac-B Exchange site). Such Confidential Information will only be disclosed to first responding emergency medical technicians.
Public Health: Trac-B Exchange may use or disclose your Confidential Information for the following purposes:
As required by law; in support of public health activities, including and not limited to: disease and vital statistics reporting and health oversight agencies, as authorized by law; in connection with certain research activities, as authorized by law.

Trac-B Exchange support your right to privacy and confidentiality. If you want more information or have questions about our privacy practices, please contact the person providing the services or our Health Services Coordinator.
Access
You have the right to review and/or receive copies of your medical information with limited exceptions provided by law.
Upon request, you may review your medical information with an onsite medical professional.
Your request for copies of your medical information must be in writing.
Restriction
You have the right to request that we place additional restrictions on our use of your Confidential Information.
We are not required to agree to these requests and will do our best to accommodate them when able.
Any agreement for additional restrictions made will be confirmed in writing and signed by a person authorized to make such an agreement on Trac-B Exchange’s behalf.
